Why it exists: choosing a nursing home blind

Few decisions weigh as heavily as moving a loved one into a nursing home. In the United States there is more than enough public information to decide well — CMS (Centers for Medicare & Medicaid Services) inspects, rates and fines every certified facility — but it sits fragmented across federal datasets and written in administrative jargon that no family is going to decode on a Sunday afternoon.

NursingHomeDex is my answer to that problem: an own product, part of my network of websites and tools, that gathers those official records and turns them into comparable profiles. A family can see on a single screen a facility's star rating, its latest inspections, how much staff it has per resident and whether penalties are piling up. That is exactly the conversation worth having before signing anything.

I designed it, built it and run it myself. There is no team behind it and no investors: it is the same data-product pattern I apply across the rest of my network and offer as a service to clients.

What you can do with it

• Search by name, city, ZIP code or state right from the homepage.

How it is built

1. Laravel architecture

• Laravel + PHP 8 on MySQL, with a schema designed to render thousands of facility, state and ranking pages without sacrificing speed.
• No generic CMS, no third-party plugins: hand-built Blade templates for every page type (profile, state, ranking, comparison, statistics).
• Lightweight frontend with Tailwind CSS and Alpine.js, compiled with Vite into hash-versioned assets.

2. Data pipeline

• Custom importer for the public CMS datasets, normalizing the federal records (ratings, inspections, penalties, staffing, beds, ownership) into MySQL.
• Monthly refresh against the official source: every CMS update flows into the comparison tool, so the figures never fall behind.
• Stable identifiers: each facility keeps its federal provider number in the URL, making profiles traceable against the official registry and immune to renames.

3. Programmatic SEO

• A dedicated URL per facility, state and ranking, each with a unique title and metadata generated from the data.
• Interlinking architecture: homepage → states → profiles → comparisons, so authority flows down to the detail pages.
• Content that updates itself: the monthly refresh keeps pages fresh in Google's eyes, a decisive factor in a niche where stale information destroys trust.

4. Performance

• Server-side rendering, no SPA and no heavy hydration: straight HTML that loads fast even on the phone of a family searching from a hospital.
• Optimized queries so rankings and state browsers are served instantly despite the size of the database.
• Minimal CSS and JS, versioned and cacheable, with no unused libraries.

Coverage and where the data comes from

The complete universe of Medicare-certified facilities: over 14,730 nursing homes across all 50 states, with the District of Columbia and territories such as Puerto Rico and Guam browsable too. Everything comes from the official CMS 5-star system — the same one regulators use — and is refreshed against the source every month.

The variety is enormous and the comparison tool reflects it as is: from large non-profit or government-run centers with more than 800 beds down to small county homes, each with its overall rating, its inspection history and its penalties in plain sight. No advertising dressed up as recommendation: just official data, organized.

The result

NursingHomeDex is a digital asset that runs on its own: the pipeline imports, the pages regenerate and the programmatic SEO captures long-tail searches from families in every state, city and facility, with no one having to write content by hand. I operate it solo, and I have replicated the same pattern — public dataset, Laravel, programmatic profiles — in other products of my network such as AgedCareScan, RehabDex and DaycareScan.
